WHERE: Lafayette Elementary
WHAT: If this is your first time attending Lafayette (recently moved to area or a Kindergarten student) this is your opportunity to complete online registration and turn in any required paperwork, along with filling out some additional forms. Required paperwork include Proof of address, Proof of Age, and Proof of Immunizations).
If your student is returning to Lafayette, you can complete online registration if you haven’t already and complete additional paperwork OR additional forms can be filled out electronically and email to office, or completed during our Open House (more information to come).
Additional forms that can be filed out electronically are the Application for Free and Reduced Meals (https://www.ode.state.or.us/apps/frlapp) and COVID-19 Testing Consent Form (https://sharedsystems.dhsoha.state.or.us/DHSForms/Served/le3560B.pdf) . BrightBytes Parent Survey / Encuesta para Padres sobre “BrightBytes”

We are partnering with BrightBytes, an educational research and analytics company, in order to learn more about our students’ school and home technology use for learning.
We are reaching out to ask you to take part in our school questionnaire. Your participation is essential to help us form a more complete picture of technology use for learning in our students’ lives, and will help your child’s school choose the right technology for their classrooms. Permission Form for Lunch From Home
Starting April 26th students will be allowed to bring a packed lunch from home, with signed permission from a parent. In order for students to bring their own packed lunch from home, the following guidelines must be followed:
Lunch packaging and carrier must be completely disposable so it can be thrown away after eaten (i.e. using a paper/plastic bag, no silverware or containers that go back and forth from home to school)
No food can be shared with another student (due to food allergies and sensitivities)

Learning Experience Survey
As we conclude the 2020-21 school year, we again invite you to please take our Learning Experience Survey. School and district leaders used survey data earlier this year to help understand and respond to what was happening in classrooms, schools, and across our district, and at the conclusion of the school year we want to check in again with our students, parents/guardians, and staff.
Your participation is completely voluntary and responses are anonymous. We want to hear your voice as we plan for the 2021-22 school year, so please take the survey linked below at your convenience! 5th Grade Drive-thru Promotion Parade!
When: Wednesday, June 16 9:30-10:30 AM
Where: The pick up/drop off lane in front of Lafayette
We are excited to celebrate your promotion to middle school, please drive or walk through the lane. Feel free to decorate your car, or make signs; at the end of the drive-thru students will be presented with a certificate.
Please direct any questions to your child’s teacher or the front office.
Chromebooks
At the conclusion of the school year we will be collecting Chromebooks/Chrometabs for all students in grades K-4 and grade 12. Students in grades 5-11 will be asked to keep their Chromebooks to be able to use for summer academics. Please note that families that would like their grade K-4 students to keep their devices to be used during the summer may elect to do so by notifying their teacher.
Next Fall, returning 5th and 9th graders will receive a new Chromebook. Beginning in the 2021-22 school year, grades K-4 will be classroom cart-based and grades 5-12 will be one-to-one device take-home based.

Experiencia de Aprendizaje
Al concluir el año escolar 2020-21, nuevamente lo invitamos a completar nuestra encuesta de Experiencia de Aprendizaje. Los líderes escolares y del distrito utilizaron los datos de la encuesta a principios de este año para ayudar a comprender y responder a lo que estaba sucediendo en las aulas, las escuelas y en todo nuestro distrito, y al final del año escolar queremos escuchar nuevamente de nuestros estudiantes, padres/apoderados y personal.
Su participación es completamente voluntaria y las respuestas son anónimas. Chromebooks
Al final del año escolar, estaremos recolectando los Chromebooks/Chrometabs de todos los estudiantes en Kínder a 4º grado y 12° grado. A los estudiantes en 5º a 11º grado se les pedirá que guarden sus Chromebooks, para poder usarlos en las academias de verano. Tenga en cuenta que las familias que deseen que sus estudiantes de Kínder a 4º grado conserven sus dispositivos para usar durante el verano, pueden optar por hacerlo notificando a su maestro/a.
El próximo otoño, los estudiantes de 5º y 9º grado que regresen recibirán un nuevo Chromebook. A partir del año escolar 2021-22, los Chromebook para Kínder a 4º grado estarán disponibles en las carretillas del salón de clase y 5º a 12º grado tendrán dispositivos individuales para llevar a casa.
