Doors will open at 7:30 AM
Breakfast will be served from 7:30 AM to 7:50 AM
Students arriving after 7:50 will need to enter through the main entrance and get a tardy slip from the office.
Early Release will be every Wednesday. Student will get out of school at 1:25 PM (With the exception of the first and last Wednesday of the school year)

WHERE: Lafayette Elementary
WHAT: If this is your first time attending Lafayette (recently moved to area or a Kindergarten student) this is your opportunity to complete online registration and turn in any required paperwork, along with filling out some additional forms. Required paperwork include Proof of address, Proof of Age, and Proof of Immunizations).
If your student is returning to Lafayette, you can complete online registration if you haven’t already and complete additional paperwork OR additional forms can be filled out electronically and email to office, or completed during our Open House (more information to come).
Additional forms that can be filed out electronically are the Application for Free and Reduced Meals (https://www.ode.state.or.us/apps/frlapp) and COVID-19 Testing Consent Form (https://sharedsystems.dhsoha.state.or.us/DHSForms/Served/le3560B.pdf) . BrightBytes Parent Survey / Encuesta para Padres sobre “BrightBytes”

We are partnering with BrightBytes, an educational research and analytics company, in order to learn more about our students’ school and home technology use for learning.
We are reaching out to ask you to take part in our school questionnaire. Your participation is essential to help us form a more complete picture of technology use for learning in our students’ lives, and will help your child’s school choose the right technology for their classrooms. Please know that all of your responses will remain anonymous to protect your privacy. The questionnaire will take approximately 5 minutes to complete.

Permission Form for Lunch From Home
Starting April 26th students will be allowed to bring a packed lunch from home, with signed permission from a parent. In order for students to bring their own packed lunch from home, the following guidelines must be followed:
Lunch packaging and carrier must be completely disposable so it can be thrown away after eaten (i.e. using a paper/plastic bag, no silverware or containers that go back and forth from home to school)
No food can be shared with another student (due to food allergies and sensitivities)

Empezando el 26 de abril los estudiantes serán permitidos traer almuerzo de casa con permiso firmado de padres. En orden para que ellos traigan su propio almuerzo tiene que seguir las condiciones siguientes:
